Swim:

bathing suit or trisuit
goggles (antifog solution?)
noseclip
wet suit
cap (usually received at registration)
waterproof watch

Transition 1:

towel
{petroleum jelly}
{sunscreen lotion}

Bike:

helmet
{cycling shorts}
sunglasses

shoes (& socks?)
jersey
number (pinned to elastic or shorts)
water bottle (ice and fluid replacement drink)
{extra water bottle}
jersey (depending on weather)
{carbo (gel or energy bar)}
pump &/or CO2 cartridges & adaptor
{extra folding tire}
spare tube or tubular tire
{tube patch kit}

Transition 2:
